 | Download and print the application form via Adobe Acrobat.

| If you are unable to do that, simply call our Customer Service Hotline at 1800 363 3333 between 8am and 10pm daily and we will mail you an OCBC-NTU Debit Card application form.

| You can also visit the OCBC Bank Branch located on the NTU Campus to pick up an application form. |
| Please submit your completed application form personally to the NTU OCBC Bank branch at:
